Please fill out this form to request an ID Card replacement for a card that has been lost, damaged or stolen. You must acknowledge that if you need an additional ID Card replacement at any time after the initial replacement there will be a $30.00 charge made to your Student Account.

When you complete this form, you will receive an email confirmation. You must print it and bring it to the Public Safety Office to receive your ID Card replacement.

Please note: If you have a Meal Plan, you must see Mary Johnson in the Student Affairs Office, 203 Sullivan Center to activate it on your new ID Card.
