Recommendation Form for a New York State Teaching Certificate
In order to be recommended by Medaille College for a NYS Teaching Certificate, each student must complete and submit the recommendation form for their specific certification area(s). Please review the NYS Recommendation Form Instructions before completing the NYS Recommendation Form. Select from the specific recommendation forms below.

Undergraduate (Bachelors) Programs

• BSED in Adolescent Education and Student with Disabilities Generalist (Grades 7-12)

• BSED in Early Childhood/Childhood Education (Birth-Grade 6)

Graduate (Masters) Programs

• MSED in Adolescent Education (Grades 7-12)
• MSED in Childhood Education (Grades 1-6)
• MSED in Literacy (Birth-Grade 6 and/or Grades 5-12)
• MSED in Students with Disabilities (Grades 1-6)
• MSED in Students with Disabilities Ext. Generalist (Grades 7-12)

• MSED in Students with Disabilities Generalist (Grades 7-12)

Advanced Certification (Post Masters) Programs

• Advanced Certification - Literacy (Birth-Grade 6 and/or Grades 5-12)

Student Teacher Profile
The Student Teacher Profile should be submitted to the Office of Student Teaching/Certification no later than February 15 for fall student teachers and September 15 for spring student teachers. Click here to view a sample student teacher profile.

Pre-Student Teaching/Student Teaching/Practicum Release Form
Students are required to complete and sign the Pre-Student Teaching/Student Teaching/Practicum Release Form prior to completing any field experiences, including pre-student teaching requirements for class as well as student teaching.

Placement Forms
Placement forms should be submitted to the Office of Student Teaching no later than February 15 for fall student teaching and September 15 for spring student teaching.
